How Do I Create a Form or Questionnaire From a Template?

To create a form, questionnaire, or contract form from a Lightfolio template, follow these steps.

Create a Form or Questionnaire

Step 1: Go to Your Form Templates

  • From the Studio Manager Dashboard, click on Forms in the vertical menu or the horizontal menu.

  • On the Forms page, click on the Templates tab.

  • Click on the form template you want to use in the My Templates section. You can also search for a form or questionnaire using the search bar.

Step 2: Create the Form or Questionnaire

  • After opening the form or questionnaire template you’d like to use, click on the green Create Form button in the upper right corner.

  • The form or questionnaire will then load in the Form Builder.

  • From here, you can edit your form’s design, enable or disable settings, view your clients’ answers to a form, and more.


Create a Contact Form

Important Note: Contact Forms cannot be added to mini-sessions, projects, or documents.

Step 1: Go to Your Form Templates

  • From the Studio Manager Dashboard, click on Forms in the vertical menu or the horizontal menu.

  • On the Forms page, click on the Templates sub-tab.

  • Search for contact in the search bar or scroll through the templates until you see a Contact Form template.

  • Click on the template you want to use.

Step 2: Create the Form or Questionnaire

  • After opening the form or questionnaire template you’d like to use, click on the green Create Form button in the upper right corner.

  • The form will then load in the Form Builder. From here, you can edit your form’s design, enable or disable settings, view your clients’ answers to a form, and more.

Note: Your created forms will appear in the Forms sub-tab under the My Forms section.
